Welcome to Conan Exiles. Originally in early access/pre-release on Xbox One and PC, it has now had a retail release as it has now come out of the 'early access' status.
With that, from the main menu, click 'Single Player', and then press 
to play Offline.

You will now be given an option for difficulty. Selected 'Civilized' which is Easy mode, and press to play.
The loading screen will take half a minute or so.
When it loads, you'll be able to make your character, starting with Male or Female.
Character Creation
You can adjust your race, religion, voice, head, face, and body features. You can choose anything you want, as your choices do not affect any achievements.
Note: If you have the free Full Nudity DLC, do NOT broadcast on Mixer with nudity on, or make Xbox Game Clips with this on, as this can lead to your Mixer Account being suspended, and your Microsoft Account being suspended, or permanently banned.
However, if you do wish to play with the Nudity DLC, you can change the nudity settings under the 'Body Features' segment of the character creation, under 'Physique'.
Having 'None' set for nudity covers up breasts and crotch for the female character, and covers the crotch for the male character.
Having 'Partial' set for nudity covers up the crotch of the female character, but leaves the breasts visible. It covers the crotch for the male character.
Having 'Full' set for nudity allows both the breasts and the crotch of the female character to be visible. It allows the genitalia of the male character to be full visible.
You are able to change the physique for both the female and male character. The physique slider makes the character more muscular the further to the right you select on the physique slider.
You can enlarge or shrink the breasts of the female character. You can enlarge or shrink the size of the penis of the male character.
When you are happy with your created character, we shall continue.
The Walkthrough
After the loading screen, you'll be placed in a desert area with cracked stone paving slabs in front of you. To the right, there is a large rock/tablet with inscriptions on. Go to it, and Interact with it with . This will bring you to Road of Kings 1/10. Hold forward on the 
and hold 
to start to climb the rocket/tablet. This will bring you to Road of Kings 2/10. When you see 'Journey step completed' press 
to drop. Now press 
to kick. This will bring you to Road of Kings 3/10. Now dodge with 
. This will bring you to Road of Kings 4/10.
Now, we're going to enable God mode, amongst other things. Press 
and select 'Settings'. Use /
to select 'Server Settings'. Press when 'General' is selected, and highlight the 'Make Admin' button. Press on it. You will now become Admin for your offline, local game.
Press to go to the tab listing on the left, and scroll down to 'Crafting'. Press on crafting, and then set the following options:
Crafting Time Multiplier: 0.1
Thrall Crafting Time Multiplier: 0.0
Fuel Burn Time Multiplier: 0.1
When you have done this, press to apply these settings. This will take you back to the menu. You will see a new option, 'Admin Panel'. Select it, and enter it with . Time to get some achievements!
You will notice a text box at the top left for 'Set Level'. Enter 10 in it, and then apply. This will unlock

Now, on the left of the admin panel, you will see checkboxes. Put a check in the two boxes marked 'God' and 'NoSprintCost'. Remove the check mark in the 'Eating' checkbox under 'Hunger/Thirst'. Press on 'Food +' and 'Water +' a couple of times each. This will bring you to Road of Kings 5/10 & Road of Kings 6/10.
Further down on the left side, put a checkmark in the 'Freeze time' checkbox. Go back to the game. Follow the obvious path between the sand dunes by sprinting with 
until you see a large creature that looks like a bat crossed with a gargoyle. Getting to this point will involve going under a broken bridge-like structure with three arches underneath it. When you see the creature, continue to the right and follow the stone path between two large pillar-like structures.
You will now see skeletons on pikes with arrows through them. Go to the right of these skeletons on the pikes to find a relatively flat part of the desert. We are now going to spawn some bosses to unlock some achievements.
Go to the admin panel again, by pressing . Make sure that you have the 'God' checkbox checked. This is invincibility, and means that you cannot die.
Highlight 'Spawn Humanoids' at the top middle of the screen and press to open up a list of every humanoid in the game. Using the search box, search frost. A list of nine humanoids will appear. Select Wildlife_Frost_Giant_Boss, and spawn him with . He will start attacking you. That's fine for now. As long as you have 'God' mode activated, you cannot die. Next, in the Spawn Humanoid search box, search for kinscourge. Only one humanoid will appear, Boss_The_Kinscourge. Spawn him with . Next, search beneath in the Spawn Humanoid search box. Only one humanoid will appear, Boss_KingBeneath. Spawn him with . You will notice a button at the bottom middle of the screen, named 'Kill All Spawned NPCs' Select it, and click it with .
This will unlock the following achievements:

Sometimes the enemies won't spawn when you tell them to. If this is the case, back out from the admin menu, move your character a little, and try again. This will fix the problem.
Next, go to the Admin menu, and this time, select 'Spawn Creatures'. Search for undead. Select and spawn Monster-Undead-Dragon with . Next, search for giant. Spawn Monster-Boss-SpiderGiant and Monster-Boss-CrocodileGiant. Next, search for sewer. Spawn Monster-Boss-SewerAbomination. Click the 'Kill All Spawned NPCs' button.
This will unlock the following achievements:

There is probably going to be a few enemy corpses around you now, so move away from them to another flat part of the dunes, and then go to the admin menu. We are going to spawn our first items. In the 'Spawn Item' search bar, search for foundation, then select 'Building' underneath the search box with . Highlight Black Ice-Reinforced Wooden Foundation and click on it 20 times with . This will give you 20 of these items. This will not put them in your inventory, they'll put them in your Item Radial.
Go to the game (out of the menus), and hold 
. Whilst holding aim 
and click . This will turn on building mode. Follow the path you came from onwards until you can actually start placing these foundation slabs down on the dunes. You'll probably be near some rocks and desert grass when you do this, probably like below:
When you can start building, click 
to 'Auto Align To Terrain', and use the 20 pieces of foundation you have to build a building foundation, probably in a 5x4 piece square. This will pop the following achievement:

Next, go to the item spawn in the Admin Panel. Click on 'Building'. Before searching for anything, you should see Altar of Set on the item list. Spawn one of those into your inventory. Next, search for bed. Spawn Double bed. Next, search for wheel. Spawn Lesser Wheel of Pain. Next, search for Blacksmith. Spawn Blacksmith's Bench.
Using your item radial on place the Altar of Set, the Lesser Wheel of Pain, the Double Bed, and the Blacksmith's Bench around where you built the foundation. The bed needs to be on top of the foundation slabs you placed. If the others aren't allowing you to place them, click to align them to the terrain, and that should solve the problem. Upon placing the bed, this will bring you to Road of Kings 7/10 and you'll unlock the following achievement:
- O Sleeper Awake
Sleep on a bed or bedroll

Next, move to the Altar of Set that you just placed, and press (not hold) to interact with it, then back out of the interaction. This will unlock the following achievement:
- The God in the Bowl
Use a religious altar

Go to the Admin Panel, and search for 'Royal' in the 'Gear' section of the Spawn Item menu. Spawn one each of the following:
Flawless Lemurian Royal Bracers
Flawless Lemurian Royal Gown
Flawless Lemurian Royal Jewelry
Flawless Lemurian Royal Sarong
Flawless Lemurian Royal Slippers
They will automatically cover up your nudity/partial nudity, and will bring you to Road of Kings 8/10.
Next, press 
and use to select the 'Feats' menu. Without leaving the menu, press and go to the Admin Panel. You should be able to see in the background, behind the 'Spawn Item' panel, the amount of knowledge points available. Next, go to the Admin Panel, and highlight the '+1 Feat' button. Start pressing it for about five minutes, as we need to have a lot of knowledge points for the next segment. Keep adding feats using the '+1 Feat' button underneath the 'Change Player Stats' on the admin panel. You want to have about 1600-1800. Once you have them, back out back to the knowledge point screen.
You will be on a menu that has a lot of item squares in green, yellow, and red. Green means you know this knowledge. Yellow means that you can spend knowledge points to learn them. Red means that knowledge is not yet available. Use your knowledge points to fill everything on the current screen that you can. Others will turn yellow as you learn things, so make those green too. Go through each recipe screen with 
until you've turned everything green.
Reminder: If you run out of Knowledge points, just go back to the Admin Panel and add more with the '+ feat' button.
When everything's green, we can continue, so come out of the Knowledge Point menu. Go to the Spawn Item search box in the Admin Panel, and search for fiber. Click on 'Gear' and you'll see Fiber Bindings. Spawn them. Next, search for and spawn Truncheon. Do not take Steel Truncheon or Iron Truncheon. It must be Truncheon. Back out of the Admin Panel, go to your Item Radial with and equip the truncheon.
Go to the Spawn Humanoid option in the Admin Panel and search for h_4_c. Spawn a Black_Hand_Blacksmith_4_Cimmerian. This will spawn Maev the Magnificent. When you attack her, you will notice she has two stat bars, one red which is her HP, and a smaller one above that which is a silvery-blue. The silvery-blue bar is her consciousness. Attack her until the silvery-blue bar is empty, and you will knock her out. When you've knocked her out, look down at her, and the prompt 'Press to drag' should appear. If it doesn't, it's because the Fiber Bindings are not on your item radial. If they're not, go to your inventory, highlight the Fiber Bindings, and hold and then place them in the radial, then press on the 'Press to drag prompt. Drag her to the Lesser Wheel of Pain, and you'll unlock the following achievement:

Next, go to the item spawn in Admin Panel and search for gruel. Select gruel, hold and then press . This should spawn 100 gruel in your inventory (or in your Item Radial). Go back into the Lesser Wheel of Pain, and move all of the gruel into the middle inventory option, and then press . You will notice the 'burn time' going down at the top. You can now move the thrall (Maev the Magnificent) from the wheel inventory (middle) to your inventory (left).
It is very likely that you will be over-encumbered, but don't worry. Move over to the Blacksmith Bench you placed earlier, press on it, and place Maev The Magnificent from your inventory into the 'Thrall' box in the middle. She's now your thrall slave and can make you items.
Go to the spawn item segment in the admin panel. Under 'resource', search for wood. Select Wood and spawn 100 into your inventory by holding and pressing . Do the same for Twine. Back out of the admin panel, go to your inventory with , and under Crafting, on the right, find 'Repair Hammer' and press to craft it. This will bring you to Road of Kings 9/10.
Next, go to the Admin Panel, and get 100 Iron Bar from the Spawn Item. Go to the Blacksmith Bench, and from your inventory, place all 80 wood, 95 twine, and 100 Iron Bars into the Blacksmith inventory (the middle section, underneath the Maev the Magnicient Thrall Listing). On the right, you want to craft Flawless Flanged Iron Mage. This will cost 12 iron. Craft it with . For me, it was the first item on the second row. When crafted, move it from the blacksmith inventory to your inventory on the left. This should pop the following achievements:

Next, equip the Flawless Flanged Iron Mage by highlighting it in your inventory, and pressing and designating a spot in the radial with . Go to the Admin Panel, and spawn any humanoid you like except for '1_none'. AltarAxe-grinder, at the top of the list, is good enough for what we need. When it's spawned, come out of the Admin Panel, and kill it with the Flawless Flanged Iron Mage. This should bring you to Road of Kings 10/10 and pop the following achievement:
- The Road of Kings

We are now pretty much done! We just need to visit a couple of places, and then get the co-op achievement!
Press and select 'Map' with /. Go to the place shown in the image below, and press to admin teleport to that exact location.
As soon as you're in the above location, you should see an alert in the middle of the screen saying that you have discovered Chaosmouth. Go to your Admin Panel, and activate 'Fly' on the left side of the screen. Back out of the admin panel. Where you teleported, you should be in front of a structure with a tall, thin gap allowing you to go inside. Go inside, and go through this room to exit out of the other tall, thin gap, whilst flying. If you're flying, you should continue on through the gap, and float over the small set of stairs going downwards in front of you. Hover around that location and the following achievement should pop:

Reach the highlands

Next, go to the map. We have another place to visit. Go to the following location:
You will spawn in a volcanic area. You will be on a volcanic path going upwards. Continue following the volcanic path upwards. When you reach the following point, the following achievement will unlock:
- Dying Embers
Reach the volcano

Next, from the exact position that the previous achievement unlocked, go into your Admin Panel, and enable Fly again. Come out of the menu, and hold until the following achievement unlocks. This will take you very high into the air.

Reach the skies above the Exiled Lands

When the above achievement pops, go back to the Admin Panel, and instead of enabling 'Fly', click the 'Walk' button, and enjoy the fall. You won't die if you still have the God mode checkbox ticked. When you land, the following achievement should pop:

Fall for 3 seconds without dying

We now have 24/25 achievements. The last achievement is a co-op achievement.
Using the map, use Admin Teleport to teleport back to where your bed is. It should be somewhere close to the image below:
Save and Quit.
From the main menu, click Singleplayer/Co-op, continue, and then press to Activate Co-Op.
When the game loads, you'll have to go back to the Server Settings and make yourself an admin again.
You now need to invite your co-op partner. When he arrives, use Fly to stand on his head. When you're standing there, go to the Spawn Item segment, and keep spawning 100s of items into your inventory. This will make you over-encumbered. Keep spawning items until your co-op partner dies from you standing on his head with so much weight. This will unlock the final achievement in the game:
- The Tower of the Elephant
Kill something by standing on its head
